Please complete the registration forms available from our information section along with the HIPAA form and bring it to your appointment.
If you have a complete and recent set of dental x-rays please bring it with you or give permission to your referring doctor for the release of your records. We reserve the right to take additional x-rays (digital) if necessary.
During our initial visit, we will review your medical history and perform a thorough periodontal examination. Please notify the office if you have a medical condition or concern (i,e. Latex allergy) prior to your appointment.
If you have been told to take antibiotic premedication before any dental appointment, kindly contact your primary care physician and ask for a prescription. Please remember to take your antibiotic 1 hour prior to your periodontal evaluation appointment.
